Man hole cover half way off sticking up in the middle of road

Resident says that water/sewer cover in the driveway is sinking in. She says that she almost fell.

Went to fill small ditch in yard at left corner of driveway (opposite mail box), found that the ditch was actually a cave in, and after clearing out weeds on the curb, can fit a shovel through the ditch to the curb.

Citizen advised there is a cave in around the sewer pipes. This has been repaired several times. Located in the 1200 block of Algona.
