cave in was reported back in February. Leak was repaired in the yard but the road still has a large dip in the middle where the sewer line runs. Someone came out and put a cone/barrel on the hole

Right up against the sewer meter cover/clean out the driveway is caving in

this storm water grate has concrete set around it which is cracked and coming apart. This leaves a hole next to the grate and could cause injury to someone. This is a storm water drain in my yard.

Resident states there is a "depression" in the street in front of this house that is 6 feet wide, at least 6 inches deep. He states there had been a sewer issue at this location in the past.
