Hole deeper than 6" in the vicinity of the water line under the roadway near this address (also reported as a pothole)

Right up against the sewer meter cover/clean out the driveway is caving in

this storm water grate has concrete set around it which is cracked and coming apart. This leaves a hole next to the grate and could cause injury to someone. This is a storm water drain in my yard.

Resident states there is a "depression" in the street in front of this house that is 6 feet wide, at least 6 inches deep. He states there had been a sewer issue at this location in the past.
