sewer cave in side driveway, leave hang tag

There is a sink hole in my yard next to the sewer drain. I have filled it in twice now, but the dirt just keeps disappearing. The dirt isn't going into the sewer itself so I don't know where it's going.

Cave in near water cut off/?Said that there is a huge hole near water cut off meter. Said that it has sunk below driveway

The manhole in the middle of the street on Indian River between Kemp Ln and Ontario Ave is caving in-West bound.

Area around sidewalk/sewer is caved in creating large holes. Problem has been progressing for 2 years. Recent service with fill dirt did not resolve problem.

Here are pictures as of 09/6/2019 I pay storm water fees for years, looks like it is time for you to spend some of that money on a fix here.. I will eventual compromise the light pole and continue to erode my yard.

Citizen advised there is a cave in behind this address. This is separate from the cave in at 4807 Bainbridge Blvd. SR# 213 093.

Citizen advised the area around the sewer or water manhole cover in the street is caving in. Before you get to White's Nursery.
